Assists with patient navigation:Assists patients in navigating clinical services provided to help overcome any barriers that may inhibit access to these services. Assists patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies. Coordinates between patients, departments and care providers:Serves as a liaison between patients and all other care providers, departments and vendors. Coordinates and schedules departmental, informational and general staff and provider meetings. Follows patients throughout the care continuum, including inpatient admissions, and collaborates with inpatient care management resources. Establishes processes, driven by community needs assessment, which address health care disparities and barriers to care for patients.
