Job Summary:
The Post-Acute Navigator is a professionally licensed health care professional with post-acute clinical knowledge who provides individualized assistance to patients, caregivers and acute/post-acute facilities. Responsible for managing patient populations with acute, chronic illness and/or multiple co-morbidities, identified as requiring additional transition of care support. The navigator will assist in overcoming health system barriers, improve patient outcomes while increasing efficiencies throughout the continuum of care within the St. Tammany Health System market.
Minimum Qualifications:
Graduate of an accredited School of Nursing or a Licensed Healthcare Professional. Must have a clinical background with strong problem-solving skills. Three to five years of experience in case management and/or therapy services in an acute care and/or post-acute setting preferred. Extensive knowledge of the healthcare system, case management, therapy services, reimbursement, federal and state regulations is required.
Current LA professional specific licensure. Current BLS certification through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to: Ochsner Health System, American Heart Association, and American Red Cross. The ILCOR course must include a manikin check off.
Strong organizational, leadership and critical thinking skills are required. Must be able to prioritize patient needs, work autonomously and within teams and have a basic working knowledge of payors, reimbursements and clinical/medical terminology and procedures. Has experience in directing and coordinating programs, projects, resources and staff. Demonstrates the professional ability to have difficult conversations with physicians, interdisciplinary teams, stakeholders, post-acute facilities and leaders. Computer skills in Word, XCEL, MS Office, Outlook, and PowerPoint.
Physical Demands:
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 50 pounds is required. Must be able to work with a moderate level of noise. Physical Effort required: Constant (67%-100%) - handling/feeling, talking, hearing, seeing Frequently (34%-66%) - lifting, carrying, pushing/pulling, reaching, stooping, crouching Occasionally (1%-33%) - climbing (stairs, ladders, etc.), balancing, crawling
