Overview Northlake Behavioral Healthcare System is seeking a Behavioral Health Case Manager to support inpatient psychiatric patients through care coordination, discharge planning, and connection to community resources. This role works closely with an interdisciplinary treatment team to improve patient outcomes, reduce length of stay, and ensure safe transitions to the next level of care.Position Position: Behavioral Health Case ManagerStatus: Full Time, Non-ExemptResponsibilities Coordinate care from admission through dischargeDevelop and manage individualized discharge plansArrange follow up care including outpatient, IOP, PHP, residential, or community servicesParticipate in treatment team meetings and track patient progressCommunicate with patients, families, and external providersFacilitate psychoeducational and skills based groups under supervisionSupport legal and community coordination as neededComplete accurate and timely documentation in the electronic medical recordAssist with crisis response and patient safety supportQualifications Bachelor's degree in Social Work required1 year behavioral health or healthcare experience preferred (inpatient or acute care preferred)BLS and CPI required (training provided if needed)Skills Strong communication and organization skillsAbility to manage a fast paced caseloadKnowledge of behavioral health systems and discharge planningEMR proficiency and attention to documentationStrong commitment to team care model and ability to collaborate effectively in an interdisciplinary settingEmpathy, compassion, and a patient-centered approach to careThis is a fast paced, collaborative role focused on helping patients transition safely and successfully back into the community.Northlake Behavioral Health is an equal opportunity/affirmative action employer.
