ESSENTIAL JOB RESPONSIBILITIESAssess for immediate and long‑term service needs.Apply the Housing First approach to serving people with housing and other service needs.Screen persons for eligibility for services and/or benefits and assist where needed.Assist an individual with goal‑setting.Identify barriers to services and seek to eliminate them where possible.Facilitate linkages to services and interface with external service agencies.Assist with applications for various services/resources.Help facilitate transportation to required service appointments.Engage high‑risk individuals (due to suicidal or homicidal thoughts/behaviors, history of hospitalizations, and/or history of addiction or risky substance use) with a focus on successfully linking them to needed higher levels of care.Participate in clinical staffing and other clinical team meetings to provide updates on care and coordinate care with other disciplines.Utilize consumer care resource funding to help people meet needs directly related to the treatment plan.Actively work with program clinicians and engage individuals in developing and implementing discharge plans, focusing on ensuring that service connections are in place.Network with community service organizations to create partnerships that can be utilized during the discharge planning process.Attempt to engage the highest-risk individuals in wellness checks for at least one year after the initial intervention.Collect and report outcome data as part of routine wellness checks with service recipients.QUALIFICATIONSBachelor's Degree in Social Service or Human Services related field.Must have reliable transportation.Must pass a background check.Drug‑free workplace; pre‑employment and random drug tests will be administered.PHYSICAL REQUIREMENTSRegularly required to operate a computer, file, and retrieve written documents, and communicate with others on the phone and in person.Frequently required to walk, sit, use hands, and lift and/or move lightweight items.Ability to move independently within the facility and community.Must be able to travel and meet with participants.WORKING CONDITIONSWork is performed in a normal working office setting that is environmentally controlled and, occasionally, out in the field.#J-18808-Ljbffr
